PITYRIASIS RUBRA PILARIS

This is a skin condition that looks very much like psoriasis to the untrained eye.  There are 5 types with a few being inherited.

Clinically it is seen as small reddish scaly papules that join together to form larger plaques.  The color is often described as orange red, which however may not be seen in brown or black skin.  These orange red plaques enlarge to form large areas on the trunks with the hallmark being “islands of normal skin” being spared.  Thickening of palms and soles appears.  The patient has no or minimal discomfort on the skin on the body but if the soles and palms are involved then they can crack up leading to discomfort.

The five types are

  1. Type 1 classical adult
  2. Type 2 atypical adult
  3. Type 3 classical juvenile
  4. Type 4 circumscribed juvenile
  5. Type 5 atypical juvenile

In types 1 & 3 the disease clears up within 2-3 years on average.

Type 4 occurs in children and improves with age.

Treatment

It is very difficult to treat and it is better to wait for spontaneous resolution.  Other treatments tried include – methotrexate, acitretin.  Topical steroids can help to reduce itching for the palms and soles 2% salicylic acid may help.
