XANTHOMAS

 

Xanthomas are specific types of skin conditions caused by collection of cells called macrophages with excess fat.

 

Different types are seen

Some are associated with disturbances in fat metabolism while others are seen without any cause.

 

Xanthelasma palpebarum

 

This is very common and seen around eyes on the eyelids as yellowish flat plaques.  This may be seen with or without increased fat levels (lipid levels) in the blood.

 

Treatment

 

Topical peels – (trichloroacetic acid)

Electrocautery

Laser

 

OTHER TYPES THAT ARE UNCOMMON INCLUDE

 

  1. Tuberous xanthomas

 

Yellowish nodules develop around elbows, knees, and around heels.  These are associated with hyper cholesterolemia. (increased cholesterol in blood)

 

  1. Tendinous xanthomas appear as nodules over hands, feet, Achilles tendon.  These are also seen with increased cholesterol in the blood.
  2. Eruptive xanthomas

 

In this multiple crops of yellow papules appear all over the body and resolve rapidly.  Another crop of yellowish red papules appear and resolve within few weeks.

These are associated with increased levels of triglycerides in the blood.  These can also be seen in diabetic patients.

 

  1. Plane xanthomas

 

As the name indicates these are flat papules and patches and can occur in any area of the body.  If the palms are involved, over the creases then it is indicative of a type of increased lipid in blood called Type iii dysbetalipoproteinemia.

 

Investigations

Blood tests will be necessary on a fasting morning sample.

 

Treatment

Diet- Decreased intake of fat products

 

Exercise -As advised by a doctor.

 

Drug treatment:-

Drugs may be prescribed by a doctor to reduce lipid levels.
